Rona Bartelstone Associates, Inc. has designed privacy practices to protect the
privacy, use and disclosure of protected health information, in accordance with recent
Federal H.I.P.A.A legislation. These practices are clearly delineated in the Notice of
Privacy Practices of this organization.
PURPOSE:
To describe the process used to disclose the privacy practices of this agency, in
accordance with Federal requirements.
PROCEDURE:
- The privacy practices of Rona Bartelstone Associates are described in the Notice of Privacy Practices.
- Rona Bartelstone Associates, Inc. also describes these practices in relevant policies and procedures that govern the
operations of the home health care organization.
- The nurse admitting the patient to service will provide the patient with the Notice of Privacy Practices.
- All new staff members are instructed, during orientation, of the privacy practices of this home health care
organization and the right of the patient to receive notification of these privacy practices upon admission
to service.
- Copies of the Notice of Privacy Practices are given to each new employee and to each patient admitted for service.
- The Notice of Privacy Practices is displayed prominently in the office of Rona Bartelstone Associates, Inc., and
made available to anyone who requests a copy.
- The Notice of Privacy Practices is reviewed annually with all employees.
- The Notice of Privacy Practices is reviewed and revised, as necessary, to reflect any changes in the privacy
practices of this agency.
- Upon revision of the Notice of Privacy Practice, the Privacy Official will ensure that all current employees,
business associates and patients receive a revised copy with notation of the changes that have been made.
- The Privacy Official will retain copies of the original notice and any subsequent revisions to the Notice of
Privacy Practices for a period of six (6) years from the date of its creation or when it was last in effect,
whichever is later.
- All employees and business associates of this home care organization are required to adhere to the privacy
practices as defined in the Notice of Privacy Practices, privacy policies and procedures and business associate
contracts.
- Protection of the privacy of a deceased patient's health information is provided for as long as Rona
Bartelstone Associates, Inc. stores or maintains this information.
- The designated personal representative of the deceased person, defined as someone with legal authority to act
on behalf of the deceased person or his or her estate, may exercise the deceased person's rights with respect to
protected health information.
