Managing the care of a loved one is a
unique challenge, taxing our resources and energy.
The challenge is often complicated by a lifetime
of relationship issues and sometimes disagreement among
family members.
At Rona Bartelstone, we provide the objectivity and professionalism
that are so important when caring for a family member. Our assessment
begins with a review of the caller’s concerns followed by
a visit with the care recipient in his or her home. During this
visit we use a variety of tools to assess the patient’s
condition and needs.
Our review addresses:
- Safety concerns related to fall prevention and independent functioning in the home
- Functional assessment to determine if assistance
is required for bathing, dressing, meal
preparation, managing appointments and
other activities of daily living
- Medication management issues
- Cognitive function to determine if thinking
or judgment are impaired and if treatment
may be necessary
- Emotional, social and spiritual issues related
to depression, isolation and other mental
health concerns
- Nutritional needs
- Whether all doctors providing care
understand the care recipient’s needs in
the home environment
The assessment is used to develop a
plan of care that will best meet the care recipient’s unique needs.
The individualized plan of care will include recommendations such
as:
- Assistive devices or home management tools
to help ensure the care recipient’s safety
- Specialty consults to assess and address
emotional and/or cognitive concerns
- Education and emotional support for
the care recipient and family
- Home care or placement in a facility
when appropriate
- Use of community services for respite, socialization
and/or emergency response
- Supervision and support of home care
staff to assure high quality care and a
rewarding relationship
- Follow up and coordination with medical
care providers to assure all medical issues
have been addressed
