Registered Nurses Submit Resume
- Administers skilled nursing care to patients as prescribed by a licensed physician and in accordance with
applicable state and federal regulations
- Services are provided to patients in their place of residence
- Must have excellent customer service skills, be able to view the person as a holistic entity and love
working in a team and with care recipients and families
CNAs and Home Health Aides Submit Resume
- Provide personal care services and assistance with activities of daily living to patients in their own home
under the direction of the RN
- Must have excellent customer service skills, be able to view the person as a holistic entity and love working
in a team and with care recipients and families
Companions Submit Resume
- Provides companionship and appropriate leisure activities to individuals in their homes
- Must have excellent customer service skills, be able to view the person as a holistic entity
and love working in a team and with care recipients and families
Care Manager Submit Resume
- Provides consultation, assessment, care planning, care implementation, quality assurance, medical advocacy,
education and empowerment for the consumer and supportive counseling to clients in their own home setting
- Must have excellent customer service skills, be able to view the person as a holistic entity and love
working in a team and with care recipients and families
- Must have extensive supervised experience in working with older adults, persons with disabilities and families
- Must have excellent knowledge of community services and benefit programs
